Abstract

The article emphasizes the right to care and the principleof hierarchy that was applied during the first phase of Covid-19 in Italy. Milestone is the document of the Italian Society of Anesthesia, Analgesia, Intensive Care and Intensive Care which had the aim of guiding medical operators in the fieldaccording to a principle of "emergency medicine", and which in fact marginalized peopleolder and more fragile.The SIAARTI document and the scarcity of care are analyzed in the first paragraph in which the object of study is whether a disaster medicine, conceived in this way, can be done correspond to a hierarchical logic of care for which the elderly were not treated according to the first served principle. In the second paragraph, some reflections on the presumed objectivity are presented in maximizing collective life expectancy, highlighting some contradictions that jeopardize the intergenerational solidarity between the different groups of the Italian population. Finally, the conclusions envisage the need forbuild a sustainable healthcare ecosystem, freeing the medical profession from decisionswhich are not the only ones responsible but which involve politics and the collective choices of citizens.
